Becoming an organ, eye and tissue donor truly shares life with others. By putting
your name on the Donate Life Tennessee Organ and Tissue Donor Registry you consent
to having your organs and tissue made available for transplantation upon your death.

Donor Sign-Up Information
All information submitted will be used only for official Registry business and will
be kept completely confidential. Your information is only shared with organ and
tissue recovery agencies that are authorized by Donate Life Tennessee to verify
this information in the case of a death of a registered donor. Hospital personnel
do NOT have access to the Donor Registry. We will not share, sell or otherwise compromise

Would you like to specify the organs and tissues you would like to donate? If you
check yes, you will be given the option to choose which specific organs and tissues
you wish to give legal consent to be recovered. It you do NOT check yes, you will
be able to donate all organs and tissues needed. These may include: heart, lungs,
liver, kidneys, pancreas, small bowel, eyes/cornea, skin grafts, heart valves, bones
and soft tissue (i.e.: tendons) or veins and arteries.
Every possible effort will be made to help save the lives of individual recipients
through your gift of organ and tissue donation. If your organs and/or tissue are
unable to be used for transplantation, they may be donated for education and research
purposes, unless you specify otherwise.

If you are between 13 and 17 years old AND have a state ID card or Driver's License/Learner's
Permit, you can join the Donate Life Tennessee organ and tissue donor registry,
but your parents will make the final decision about organ and tissue donation at
the appropriate time.
