Be sure to mark "YES" to organ donation EVERY time you renew or make changes to your driver's license to remain on the Registry. If you do not indicate "YES" to organ donation on your driver's license renewal, your name will be removed from the Registry.

Before you sign up please take a few minutes to make sure you understand what it means to be on the Registry:

Becoming an organ, eye and tissue donor truly shares life with others. By putting your name on the Donate Life Tennessee Organ and Tissue Donor Registry you consent to having your organs and tissue made available for transplantation upon your death.

• Examples of organs for life saving transplants include heart, liver, pancreas, kidneys, lungs and small bowel.
• Examples of tissues that could save or enhance someone's life include eyes/corneas, heart valves, bones and skin grafts.

If you have previously registered on this site, you may modify your records by going to: Update My Profile.

Specific Donations
Would you like to specify the organs and tissues you would like to donate? If you check yes, you will be given the option to choose which specific organs and tissues you wish to give legal consent to be recovered. It you do NOT check yes, you will be able to donate all organs and tissues needed. These may include: heart, lungs, liver, kidneys, pancreas, small bowel, eyes/cornea, skin grafts, heart valves, bones and soft tissue (i.e.: tendons) or veins and arteries.

Every possible effort will be made to help save the lives of individual recipients through your gift of organ and tissue donation. If your organs and/or tissue are unable to be used for transplantation, they may be donated for education and research purposes, unless you specify otherwise.

Terms and Conditions
If you are between 13 and 17 years old AND have a state ID card or Driver's License/Learner's Permit, you can join the Donate Life Tennessee organ and tissue donor registry, but your parents will make the final decision about organ and tissue donation at the appropriate time.

By submitting this registration, I affirm that I am the applicant described on this application and that the information entered herein is true and correct to the best of my knowledge. This registration will serve as a document of gift as outlined in the Tennessee Uniform Anatomical Gift Act. A document of gift, not revoked by the donor before death is irreversible and does not require the consent of any other person. It also authorizes any examination necessary to ensure the medical acceptability of the anatomical gift.
